Otaru Station, the gateway to Otaru tourism, opened in 1903 (Meiji 36) as "Otaru Chuo Station" of Hokkaido Railway. It changed its name repeatedly to "Takashima Station" and "Chuo Otaru Station" before finally becoming Otaru Station in 1920 (Taisho 9), taking the name of the former Otaru Station (current Minami Otaru Station). The current station building is its third incarnation.

Due to the gradient of the railway tracks, the station was built on a hillside, and the area in front of the first and second generation station buildings was a steep slope, which hindered the passage of horse-drawn carriages and other traffic. The current station building, completed as a reinforced concrete structure in early 1934 (Showa 9), has its platforms located on the second floor level at the rear of the main building.

The exterior featured a modern, symmetrical design modeled after Ueno Station, which had opened two years prior. This style was also adopted by Chiayi Station in Taiwan, which was under Japanese rule at the time, and was a typical structure for urban station buildings designed by the then Ministry of Railways.

The former station master's office on the concourse retains its original appearance and is open to the public during events. The "Mukai Kane" bell at the front of the station building was rung as a train arrival announcement from the station's opening until around 1965 (Showa 40).

Furthermore, because Yujiro Ishihara, who had close ties with Otaru, disembarked at Platform 4 when he visited Otaru for filming, Platform 4 has been affectionately known as "Yujiro Home" since 2003 (Heisei 15), and a life-size panel of him has been installed there.
